Survey on proposals for urgent and emergency care services

We are your local NHS in Southport, Formby and West Lancashire and we are asking for people’s views on two options for changing local urgent and emergency care services to help meet the challenges and goals outlined in our consultation booklet. Both options involve bringing adult and children’s accident and emergency (A&E) services together on a single site at either Southport or Ormskirk hospital.


We recommend reading the booklet before completing the survey or alternatively reading the summary version (seven minute read). There is also an easy-read version.


This survey is for everyone. However, some questions may be harder to answer than others depending on your experience of using NHS services, of working with or for us, or on your broader knowledge of the NHS in general. We provide some additional information to help inform your answers. If you don’t feel comfortable answering a question you can select ‘don’t know’ and continue. Or if you need help finding or understanding the information provided, please get in touch and let us know how we can support you.  

Decisions will only be taken once we have heard a wide range of views. We need to be confident that proposals are based on all available evidence and consider the views and opinions of people who use and rely on our services.
